Refer your patients

​You can refer patients to me via email (HIPAA secure) or fax. 

• Please include patient demographics and a summary paragraph that includes a clear description of how you and your patient hope I can contribute to their care.

• Please be sure to clearly explain to your patient that I am not a primary care doctor, and that they will be expected to maintain a relationship with a PCP while working with me. I look forward to collaborating with you and your patients!

After sending your referral, the patient should schedule a free Meet & Greet with me to discuss whether we will be a good fit for each other.​​​
